What is a Care Plan?
Your Dovida Care Plan, sometimes referred to as a Support Plan, is a detailed document prepared by your Care Manager in conjunction with you, your family members, health care professionals and anyone else who is part of the individual’s Circle of Care.
It describes the details of the care that is to provided, and it is used by appropriately trained caregivers to deliver the right care, at the right time, based on their clients’ unique requirements.
Care Plans are regularly reviewed by Care Managers, and they can be adjusted as we get to know you, ensuring that you’ll always live Your Life, Your Way.

How do I get started with Care Plan Management?
First, contact your local office, who will arrange a free, no-obligation care consultation.
A Care Plan will then be created, and your Care Manager will work closely with you to start organising your care in a way that works for you. This will include creating a visit schedule.
